2.Phones Management:a)Primary responsibility for all incoming calls to the department, including all voice mail messages. Answers patient phone calls utilizing UT Southwestern PURE concepts.b)Utilizes department protocols for problem solving as necessary to meet patient's needs.c)Documents and routes patient communications appropriately following all EMR and other medical record documentation standards.
3.Builds and maintains positive working relationships with the patients, care coordinators and certified community health workers.
4.Supports patients through facilitation of community resources as requested and under the direction of the certified community health worker or nurse care coordinator.
5.Continuously expands knowledge & understanding of community resources.
