Experience and Education


Master degree in Social Work with at least one (1) year of experience in a health care setting preferable with homebound patients._**Current license is required by the Texas State Board of Social Work Examiners. Temporary license is acceptable during the first 90 days of employment.**

Job Duties


Ensures the delivery of quality social service for patients by identifying patient needs and developing social service programs to meet those needs.
Participates in development and update of Plan of Treatment.
Evaluates patient and home environment to determine social and phychological factors related to patient's health problems.
Prepares and submits to agency psychosocial histories, clinical and progress notes. Observes and reports on patient/family reaction to treatment.
Counsels patient and/or family in understanding, accepting and adjusting to patient's health problems and emotional care.
Identifies, informs and refers patient/family to available community resources.
Coordinates discharge planning.
Interprets to the community the role of social work in the Agency's programs.
Participates in planning, development and delivery of staff in-service education programs.
Participates in Continuing Quality Improvement Program. Monitors adherence to policies, procedures and protocols.
Performs other ralated duties as assigned or requested.
