Experience and Education


Graduate of an accredited School of Nursing; Palliative care nursing education and/or recent clinical experience in palliative care an asset; Registered as a RN or ANP. Three years' palliative/hospice experience.

Job Duties


1.Provides patients with life threatening illnesses and their families' continuity of care through the illness trajectory, between home, hospital and long term care facility. Monitors and evaluates care plan as necessary. Advocates for change where systemic barriers or gaps in palliative care exist.
2.Receives referrals and conducts appropriate assessments. Coordinates access to other services such as regional palliative care services, physical therapy, pharmacy and nutrition.
3.Arranges admission to or delivery of required services of palliative care program.
4.Provides or arranges for anticipatory grief counseling. Facilitates team and family conferences.
5.Participates in the development of communication strategy for palliative care patients.
6.Coordinates resources such as equipment, medications.
7.Coordinates staff support and volunteer services.
8.Coordinates the interdisciplinary team and facilitates the integration of the palliative care consult team. Works with hospital, home care and long-term care staff to integrate palliative care services with other services. Coordinates movement of patients between care settings when required.
9.Participates on committees as appropriate.
10.Provides advanced assessment skills for more complex palliative care.
11.Educates and counsels patients and families about the disease process, symptom management and care choices.
12.Acts as a palliative care resource for agencies and organization providing palliative care (Home Care, Homes for Special Care, etc.).
13.Manages the distribution, collection and evaluation of palliative care satisfaction surveys.
14.Assures the documentation of DSRIP-required performance elements.
