POSITION SUMMARY
The Department of Physical Medicine & Rehabilitation has a new employment opportunity for a candidate to join their team as a Social Worker. The mission in the PM&R Department is to optimize quality of life and function through excellence and innovation in patient care, research, and education. PM&R physicians, AKA Physiatrists, treat a wide variety of medical conditions affecting the brain, spinal cord, nerves, bones, joints, ligaments, muscles, and tendons. We focus on non-surgical diagnosis and management of conditions to improve our patient’s mobility and less difficulties in cognition and communication. The ideal qualifications and duties for this position should include but not limited to the following:
· Licensed Clinical Social Worker (LCSW) preferred.
· Will provide counseling services at least two (2) days per week individually and in group settings.
· Provide PM&R patients assistance with the following:
o Community resources include meal and transportation resources.
o Job related resources.
o Assistance with adjustment to disability
o Post-traumatic stress disorder management
Shift: 8-hour days, flex shift
EXPERIENCE AND EDUCATION
REQUIRED
· Graduate of an Institution certified by the Council of Social Work Education with a Masters in Social Work
· The current license is required by the Texas State Board of Social Work Examiners.
· A temporary license is acceptable during first 90 days of employment.
· BCLS is not required in transplant departments.
· Prefer 2 years’ experience.
· Good general health and stress coping ability.
· Full mobility and ability to participate in vigorous activity.
· Ability to speak and hear, to allow discussions with physicians, patients, and other members of the healthcare team.
· Possess near vision for accurate reading of computer screens and recording on patient charts.
· Must be able to walk reasonable distances to cover multiple nursing units.
JOB DUTIES
· Participates in multidisciplinary rounds and proactively determines patient needs for discharge transitions including transportation, DME, LTAC, SNF, home health agencies and hospice.
· Utilizes crisis intervention and problem solving skills to facilitate and enhance patient and family’s ability to meet acute and ongoing medical and psychosocial needs.
· Demonstrates ongoing familiarity with community resources to facilitate positive patient and family outcomes agencies to assure appropriate client access.
· Maintains effective documentation including tracking and referral information; consistently meets department and UTSW documentation standards. Identifies and reports situations or cases to appropriate governmental agencies as legally required and in compliance with organizational policies and procedures.
· Serves as an active participant of the health care team, communicating effectively with other health care providers, patients, families, other hospital personnel, and visitors.
· Identifies the need for professional growth and seeks appropriate development opportunities.
· Provides education to staff, patients and community resource affiliates.
· Maintains system for tracking placement of patients after discharge on monthly basis.
· Has knowledge of Department and Unit Performance Improvement programs and is aware of expected participation.
· Identifies the need for professional growth and seeks development opportunities.
