PRN Physical therapy services for home care patients.

Experience and Education


Undergraduate degree and one (1) to two (2) years in Physical Therapy is required. **Cardio-Pulmonary Resuscitation, drivers license and licensed Physical Therapist is required.** Preferred: Home health services

Job Duties


Directs and aids patient in active and passive exercises, muscle re-education, and functional training using pulleys, weights, steps, and inclined surfaces.
Provides instructions in therapeutic procedures to be continued by patient and family.
Adapts conventional physical therapy techniques to meet needs of patients unable to comprehend verbal commands or voluntarily carry out regime of therapeutic exercises.
Designs physical therapy programs and sets goals for each patient.
Evaluates, records, and reports on patient's progress for review by members of care team.
Maintains patient records and charts to ensure accurate record of treatment.
Monitors delivery of physical therapy services in adherence to agency's clinical standards.
Participates in Continuous Quality Improvement Program.
Performs other duties as assigned.
