UT Southwestern Medical Center has a new opportunity available for the of Social Worker – Home Health. This is a part-time position and the ideal candidate will provide coordinated care in the home to clients and assists with creating patient centered action plans to promote effective education, self-management support, and adherence to the PCP provider’s plan of care facilitating access and timely healthcare delivery. You will see patients who require skilled care after an in-patient hospital visit and determined what additional services are required such as meals on wheels, care plans, etc. Services typically last 9 weeks once assigned. The ultimate goal is to assist the patient to independence within the stated time frame.

EXPERIENCE and EDUCATION:  

• Master degree in Social Work with at least one (1) year of experience in a health care setting preferable with homebound patients.
• Current license is required by the Texas State Board of Social Work Examiners.
• Temporary license is acceptable during the first 90 days of employment.

JOB DUTIES:  

• Ensures the delivery of quality social service for patients by identifying patient needs and developing social service programs to meet those needs. 
• Participates in development and update of Plan of Treatment. Evaluates patient and home environment to determine social and psychological factors related to patient’s health problems.
• Prepares and submits to agency psychosocial histories, clinical and progress notes.
• Observes and reports on patient/family reaction to treatment.
• Counsels patient and/or family in understanding, accepting and adjusting to patient’s health problems and emotional care.
• Identifies, informs and refers patient/family to available community resources.
• Coordinates discharge planning. Interprets to the community the role of social work in the Agency’s programs.
• Participates in planning, development and delivery of staff in-service education programs.
• Participates in Continuing Quality Improvement Program.
• Monitors adherence to policies, procedures and protocols.
• Performs other related duties as assigned or requested. 

KNOWLEDGE, SKILLS & ABILITIES: 

• Work requires good general health and stress coping ability.
• Work requires full mobility and ability to participate in vigorous activity.
• Work requires ability to speak and hear, to allow discussions with physicians, patients, and other members of the healthcare team.
• Possess near vision for accurate reading of computer screens and recording on patient charts.
• Must be able to walk reasonable distances to cover multiple nursing units.
