JOB SUMMARY
Efficiently manages patient transfers with minimal oversight, ensuring smooth navigation through designated workflows with critical thinking, while expediting priority as condition dictates. This position works under the general supervision of the Patient Flow Assistant Manager of Bed Control/Transfer Center.

EXPERIENCE AND EDUCATION
Required

• Education
  Graduate from an accredited Nursing program

• Experience
  5 years of acute care nursing experience required and
  Requires previous experience in Progressive Care, ICU, and/or ED

JOB DUTIES

• Efficiently take clinical information from outside hospital to facilitate the transfer process.
• Review patient medical records to ensure accurate information is provided to UTSW Providers.
• Establish and maintain ongoing communication with all stakeholders while securing transfers and obtaining priority levels.
• Provide all critical information to Provider regarding Level of Care and EMTALA Status of Patient.
• Appropriately follow up on all clinical information that needs to be provided to Provider prior to accepting or giving MOT
• Run and completes financial clearance when incoming status is inpatient
• Escalate to Insurance Verification team when unable to financially clear.
• Ensure authorization is obtained on all planned procedural patients prior to transfer
• Escalates deterioration in patient condition and ensure provider is aware of any changes prior to giving acceptance or while enroute.
• 1Use Critical thinking and Nursing Judgement to identify a change in patient condition that needs to be escalated to provider. Ensure that it is escalated to provider to determine a change in priority and acceptance of patient.
• Ensure that any changes in patient condition are updated to provider prior to MOT being given.
• Request a Return Transfer Agreement (RTA) appropriately. Ensure that any changes in patient condition are updated to provider prior to MOT being given
• Notifies Administrative Coordinator or Manager concerning admissions that do not meet the hospital’s criteria, is Out of Network or does not follow the outlined guidelines established by the Transfer Committee.
• Continually seek new ways to improve processes, increase efficiency, and take the initiative to communicate recommendations to Assistant Nurse Manager or Manager.
• Identifying and resolving any issues that arise during the transfer process, ensure efficient effective communication and collaboration throughout the transfer process.
• Ensure that transfer encounters are linked to procedural cases.
• Communicate effectively with patient, provider and receiving units throughout the Direct Admission Process.
• Able to document all interactions timely and accurately to ensure that patient is safely admitted to unit with all necessary information.
• Performs other duties as assigned.
