JOB SUMMARY
Provides speech therapy services to patients in their homes to improve communication, swallowing, and cognitive function. Evaluates needs, develops treatment plans, and works closely with healthcare teams. Requires licensure, clinical expertise, and strong interpersonal skills.

JOB DUTIES
Evaluates patient’s home environment and communicative status of patient.
a. Documentation reflects the identified problems or disability, interventions, communications, and patient responses and is supported by the Plan of Care.
b. Necessary forms are completed in a timely manner.
c. Documentation is complete and reflects compliance with agency policies.
Develops plan of care (POC/POT) in coordination with physician (when required).
a. POC/POT is based on initial assessment, speech and language disorders, and medical diagnosis, and is completed in a timely manner.
b. POC/POT is relevant to the patient’s current needs.
c. Patient, family/caregiver, and health care team members are involved in implementing the POC/POT.
d. Outcomes are evaluated and suggestions for change made accordingly.
Provides patient care and coordinates the care with other disciplines and team members with the assistance of the Clinical Therapy Manager.
a. Patient care, as ordered by the attending physician, is prioritized and evaluated on an ongoing basis.
b. Appropriate methods of assisting the patient in improving, correcting, and/or accepting his/her disability are taught to the patient, family/caregiver, and health team members as necessary.
c. Patient care is performed within speech pathology policies and procedures in accordance with rules, regulations, codes of ethics, guidelines, and codes in federal, state, and local laws concerning the practice of speech therapy.
d. Proper infection control, hazardous waste, HIV/HBV, etc. procedures are used in accordance with agency policy.
Coordinates patient care with other disciplines and team members.
a. Observed changes in the patient’s condition (progress and/or regression) and reactions to treatment are reported to the physician in a timely manner.
b. Team conferences are participated in as required per agency policy and constructive and relevant information is provided.
c. Daily reports are given to the Clinical Therapy Manager regarding patient problems and changes in the POC/POT.
d. Communications are timely, effective, and courteous.
Documents care and interventions.
a. Necessary forms are completed in a timely manner.
b. Documentation reflects the identified problems or disabilities, interventions, communications, and patient responses and effectiveness of treatment provided.
c. Documentation is legible and complete, and reflects agency policies.
d. Appropriate written reports of the patient’s progress on the appropriate forms are submitted to the physician and agency at the designated intervals of every 60 days and at discharge of the patient.
Participates in the implementation of the Quality Improvement Plan.
a. Understanding of role in the QI plan can be verbalized.
b. QI activities are incorporated into the performance of job assignments.
c. QI activities as assigned by the Clinical Therapy Manager are completed within the given time frame.
Maintains a safe and healthy environment.
a. Instructs patients on safety at home.
b. Uses universal precautions.
c. Maintains PPD and CPR per agency policy.
